1. Scholarship applicant must be a full-time student (carrying
a minimum of 12 hours per semester) in an approved Kentucky
institution for higher education paying instate tuition (as
established by the institutions registrar's criteria).
2. The scholarship applicant must be a student who is structuring
their academic program in the direction of a career in public
or private sectors of environmental health practice. Students
enrolled in one of the following programs are eligible to apply:
* 1. An accredited environmental health science curriculum.
* 2. Dairy Science and /or Food Technology curriculum.
* 3. Community Health with and emphasis in environmental health.
3. Applicant must attach a complete grade transcript of courses
from all previous semesters, plus documentation substantiating
current enrollment.
4. Applicant must submit a cover letter from their Academic
Advisor/Professor from their area of study and from the Institution
from which they are enrolled.
5. Applicant must have obtained a grade point average of no
less than that of a "B"(3.0) in their major and shall
have completed not less than 9 credit hours towards their major.
6. Applicant must have submitted their application postmarked
no later than April 30th
7. The Scholarship will be paid to the institution in the receivers
name for application toward the fall and spring semester.
8. No individual is eligible to receive the Scholarship more
than once.
9. If there is more than one qualifying applicant, an alternate
shall be selected by the Scholarship Committee in the event
the primary choice should not enroll during the fall semester
